The jackathome.com website (this "Site") and the semen testing service ("Service") are provided by Care For There, LLC (collectively, "Company", "we", "us" or "our"). The following sets forth the Privacy Policy ("Privacy Policy") of Company and its affiliates with respect to information collected through the Site or otherwise from our customers, and is incorporated into and made a part of the Company Terms and Conditions, available at the Site. This Privacy Policy describes how information about you is collected, used and disclosed by us when you sign up for and use the Services and it provides other important privacy information, describes when and how we may change this Privacy Policy, and tells you how to contact us with any questions or comments.

- HIPAA Rights
When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you. You may request an electronic or paper copy of your records and other health information in possession of the Company. We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee to satisfy these requests. You can ask us to correct health information about you that you think is incorrect or incomplete. We may say “no” to your request, but we will tell you why in writing within 60 days. You can ask us to communicate with you in a specific way (for example, via secure email, home or office phone, etc.). We will honor all reasonable. You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect the services, we provide you. You can ask for a list (accounting) of the times we have shared your information for six years prior to the date you ask, who we shared it with, and why. We will provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months. You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly. If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has this authority and can act for you before we take any action. You can complain if you feel we have violated your rights by contacting us. We will not retaliate against you for filing a complaint.
For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions. In these cases, you have both the right and choice to tell us to: Share information with your family, close friends, or others involved in the services we provide to you. If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest. We may also share your information when needed to lessen a serious and imminent threat to health or safety. - California Residents
California has a data protection law called the California Consumer Privacy Act (CCPA), as amended, and this Section 109 includes information we are required to disclose under that law. This section applies to you only if you are a California resident.

Verification of your identity
We have to verify your identity before we can give you access to, or delete, your personal data. We may use information that you have provided to us, or information from a governmental identification, or through two step verification (where available) to verify your identity. If we can’t verify your identity, we’ll unfortunately have to deny your request. This is to protect you.
What to expect once you submit a request?
We will confirm receipt of the request within 10 days via the email address you provide. Before we honor your rights to request information or delete information, we must verify your or your authorized agent’s identity. In certain cases, we may require you to provide additional information in order to verify your request.
